Radical prostatectomy and genitourinary toxicity in patients with prostate cancer and previous postoperative radiotherapy. We compared the incidence of genitourinary toxicity after radical prostatectomy (RP) in patients with and without preoperative radiotherapy (RT). We studied 251 patients after RP from November 1987 to December 1997 for prostate cancer. Twenty-three patients (9.1%) had received RT before RP. The median radiation dose was 75.6 Gy (range, 56.4 to 76 Gy). Followup was 9 (range, 1 to 20) months for all patients and 27 months (range, 18 to 43) for living patients. The median serum prostate-specific antigen levels before RT, after RP, and at followup were 0.4 ng/ml (range, 0.07 to 5.37 ng/ml), 0.7 ng/ml (range, 0.07 to 11.7 ng/ml), and 0.4 ng/ml (range, 0.07 to 11.7 ng/ml), respectively. Genitourinary toxicity, according to the 1994 modified criteria of the RTOG, was grade 0 or 1 in 94.3% of the patients without RT, 76.3% of patients with RT for less than 6 months, and 52.2% of patients with RT for more than 6 months. The 2-year risk of genitourinary toxicity was 3.4% in patients with RT for less than 6 months and 13.2% in those with RT for more than 6 months. There was no significant difference in genitourinary toxicity after RP between patients with or without RT (P = 0.13). The 2-year risk of genitourinary toxicity after RP with a dose of 75.6 Gy was 3.4% and after RP with a dose of 56.4 Gy was 6.5%. The mean prostate weight was 51.7 +/- 35.5 g in patients without RT and 45.1 +/- 22.7 g in patients with RT. The mean prostate volume was 54.9 +/- 30.7 ml in patients without RT and 35.3 +/- 15.8 ml in patients with RT. There was no significant difference in these parameters between patients with or without RT (P = 0.26, 0.38, and 0.33, respectively).
